FOR CURRENT 2024 LEARNING COLLABORATIVE MEMBERS
Thank you for participating in the 2024 Collaborative! Please complete this ZS Organizational Self-Study by March 7, 2025. This will help us measure any impact or outcomes across partners that are associated with participating in the Collaborative. At the end of this survey you will have the option to pledge to continue with us in the 2025 Collaborative. If you elect to continue, your responses will also be used to identify highest areas of need across participating partners and guide the session topics for our 2025 Collaborative.

All of your responses are completely confidential and will not be shared or published with any other agencies or entities. The data entered here will be used to identify highest areas of need across participating partners and guide the session topics for our 2025 Collaborative. At the completion of the 2025 year, you will be asked to complete another Organizational Self-Study to help measure any impact or outcomes across partners that are associated with participating in the Collaborative.

Those pledged to participate in 2025 will receive a calendar invitation for bimonthly 2nd Wednesday meetings between 12:30-2pm EST, beginning May 14, 2025 (and running July, September, November, January, March).
